Why did Nicolas Cage get angry and start throwing the table at the casino? Was is it something the waitress said?

The waitress reminded him of his ex-wife. He was hallucinating because of the gratuitous amount of alcohol his body had absorbed. Notice while he was being escorted out by security, he screamed, "I am his father!", thus, referring to his son.

He looked drunk and almost passing out. The waitress probably "cut him off". Vegas may have a reputation of taking advantage of drunk people, but when you are that drunk they will cut you off. Try it and see. I work in a casino, I have seen it.

Have you noticed how fake was waitress' falling? Nobody pushed her, she just... jumped off :-)

Exactly. She told him they couldn't serve him anymore, and he got pissed.
