During the making of the 2nd movie, Keiko was extremely ill - to the point where vets were unsure if he'd die before the tank that was being built for him in Oregon would be ready for him. He was in no condition to be used for a movie, much less put into the ocean and exposed to potential germs and diseases that his extremely fragile immune system wouldn't be able to handle, or expose wild populations to the papilloma virus that he was suffering from.
During the making of the 3rd movie, Keiko was being moved to/recovering in the specially built tank in Oregon. Again, he was in no condition to be put into the ocean for the same reasons.
Keiko wasn't put into the ocean at all until September of 1998 when he had sufficiently recovered and was moved to a sea pen in Iceland for rehabilitation and eventual release.
