How did Carlito manage to get the bullets out of the .38 during the hospital room scene?

I have re-watched this scene a bunch of times and i still do not understand how he managned to get the ammo out of the weapon in this scene, is there something i am missing?

Didn't Carlito bring the gun with him?

He probably took the bullets out before entering the room... he probably wasn't even sure if he should or shouldn't give him a loaded gun a short time before going in... and had conflicting thoughts about how the Tony-T thing went down and his family coming for them.

Kleinfeld had the gun stashed under his pillow, he reaches for it when Carlito walks in and Carlito grabs it and is like "what's this?"

Yeah, looked for that scene... I believe you do have an argument, but there can also be a defense.

DePalma brilliantly keeps the camera not focusing on Carlito's hands when he's handling the gun while standing at the end of Kleinfeld's bed... Carlitos kept "giving him advice" but he was actually distracting Kleinfeld while being subtle and taking the bullets out, a little "magic" like he did earlier during the pool scene, remember that this guy is a legend of the streets and most likely could be doing whatever he's doing to the gun blindfolded, he's CARLITO BRIGANTE MOTHERF*CKER TO THE MAX!

I just consider it a plot hole

The same thing happens in "Young Guns" in the saloon scene when Emilio Estevez(Billy The Kid) handles the man's gun who says he's gonna kill BTK(not knowing that was him right there).
