When Nick goes to the hospital to learn more about Beth's past, he discovers that her husband died 5 years ago, and he is later told that there was talk of "his wife having a girlfriend." Can someone explain this whole thing to me? Did Catherine kill Beth's husband? Were Beth and Catherine together? This scene confused me. It led me to believe that the "twist" was that Beth and Catherine were longtime lovers and had planned all of this together.

I think Catherine was obsessed with Beth right from College. Catherine copied/imitated her, wanted to be her and in a relationship with her, but Beth rejected Catherine because she was weird/psycho and called her out on it. Catherine played the 'victim' and reported Beth for harassment which is a common strategy with stalkers.
Catherine kept tabs on Beth throughout the years planning revenge. She murdered Beth's ex-husband 5 or 6 yrs earlier, she murdered Boz, who might have had a sexual fling with Beth, she murdered Nilson because he was snooping and could expose Catherine's plans.
Catherine discovered Nick was sleeping with Beth and found out all the dirt on him, and he became the next target. Catherine asked Roxy to kill Nick and her car was tampered with so she would die while attempting to run Nick over. We can assume she tampered with her parents car as well, causing their deaths.
Catherine then killed Gus and set Beth up for all the murders. She knew Nick was with Gus and hoped he would either kill Beth or arrest her. Beth had complained to Nick that the lock on her door was broken. The evidence was planted so if Beth survived, she would be charged and sent to prison. Even better if she were dead, the case is then closed.
Catherine planned to kill Nick with the ice-pick but chose not to when he said he wanted to spend his life with her.
Basically, Catherine kills her lovers if they attempt to leave. She was obsessed with Beth for at least 8 yrs. Nick's fate will probably be the same but he's going in with eyes open.
According to Sharon Stone, Nick's fate was the same. Remember, in the sequel, Catherine was questioned whether Nick was still alive and after a pause, she replied, "No." We don't know any more about Nick's fate except that in the sequel he is dead. Sharon Stone elaborated on this, by basically saying Catherine killed him.
I totally agree with your theories as well. Catherine was a malevolent person, who was highly obsessive. She liked to toy with people but if there was anyone Catherine was in love with, it was Beth. Beth's rejection of Catherine destroyed whatever sanity she had left. After that, she became consumed with revenge and wanted nothing more than to destroy her, killing or hurting anyone who got in her way.
