Maybe this is just me, but it has always bothered me. Why do they have Macaulay Culkin on the cover without glasses, when during the whole movie, he wore glasses?? I don't get it, did they think the audience wouldn't recognize him on the movie cover with the glasses on? (By the way, I am referring to the VHS cover)
probably ^, i remember he lost his glasses during the bee attack, so it may symbolize thomas j. didn't need them in the after life, so the cover was like him ... i don't know where im going with this but you kinda get what i mean

I always got the impression that the image was after his death and Vada was reminiscing about him. She seems to be remembering and laughing while Thomas J.'s image is surrounded by white and he no longer needs the glasses in death. That's what I've always thought anyway.
My guess is that the studio execs thought that people might not recognize Culkin with glasses on and since he was such a big star in the early 90's they decided to put him on the cover without glasses so that people will know it's him and it will draw more people in to seeing the movie. I know it doesn't sound as poetic as the last couple of replies but it's the most likely case.

Now, many years later, my theory is: this was a picture someone took of the two kids goofing around and it's natural. They probably liked it so much they used it for the cover. Look at their clothes, nowhere in the movie are they dressed like that, and they don't even look like they're in the time period the movie's set at. And then there's the obvious fact that Macaulay isn't wearing glasses.

Great theory, another evidence for your theory is the hair- look at Macaulay Culkin hair in the poster and look on a photo of him played Thomas. entirely different people.
