Lilla is dead. She dies about 88% of the way through the book, while her counterpart in the film, Eve, survives to the end. Of course, that is the present day Eve. Eve's precursor dies in a hallucination.

Some people have remarked upon the low body count in the film. It may not be as violent as some films, but the body count in the film is certainly higher then the book's, which can be seen in the comparison below.
