Why did they recycle the Rambo theme for the trailer?

Trailers are produced when the movie isnĀ“t finished. So the production companies or the dirtributers use other music score, where they got the rights for. You find this very often.

Could it also have something to do with how Jerry Goldsmith did the scoring for both Rambo and Extreme Prejudice.

The advance teaser is usually scored with temp or source music, as the score for the finished product usually hasn't been laid down yet. In this particular instance, both RAMBO: FIRST BLOOD PART II and EXTREME PREJUDICE were produced by Carolco, both were scored by Jerry Goldsmith, and Goldsmith's score for RAMBO is among the best scores of the 1980s (and certainly the best militaristic action movie score), making it a natural to use it.

Strangely, the theatrical trailer I remember seeing didn't use RAMBO cues, OR Goldsmith's finished score for EXTREME PREJUDICE, but damned if I can place exactly the hell it's from.

If it had been made from about 1993 onwards, it likely would have been either Thomas Newman's score for THE SHAWSHANK REDEMPTION, James Horner's GLORY, or Michael Kamen's ROBIN HOOD score, since I could probably count on one hand the numbers of trailers I've seen in the last ten years that DIDN'T use one at least one of those...

Exactly, not like it's anything new. Basil Poledouris' Anvil Of Crom (from Conan The Barbarian) is used on more than a few "sword and sandal" films, specifically Gladiator.
