Miami, FL in Casino Royale, too, but pretty much just the airport.

Licence to Kill for Felix Leiter’s wedding. Forgot where, though.

The first Bond film where you see Bond in America is Goldfinger.

The first Bond film where you first see him actually in America is Diamonds Are Forever. Connery was never in Miami or Kentucky in Goldfinger.
