They made a point to mention that Angela's body or boobs were small and this is understandable for a girl's age. But since Angela is a boy, she shouldn't have any developing breasts right? Angela may have been small but you can clearly see that the actress was developing around this time. The only way for a boy to have small boobs that looked like Angela's on the film is if he was fat, Angela was thin. They should've taped it imo, because even though I entered this film knowing about the twist, the actress' development was clearly showing through her shirt, and this reassured me that she must be a girl and not a boy.

She would probably have breasts if she was on hormones. I don't know if they were widely available back in 83.

The bigger issue would be that around puberty, she would probably begin to get facial hair.

But puberty in boys ranges from 12-18. Look at the boy who played Angela's cousin Ricky, the actor was 17-18 when he played the role but looked 14/15. Ralph Macchio in The Karate Kid was the oldest of all the kids in the film, he looked the youngest and was 21 but looked 15, in The Outsiders he was 19-20 but looked 14. So shaving could've started much later for some boys, I know my cousin at 21 who looked fully mature by then but only had 3 stands of facial hair.

I've read a book about prisoners in concentration camps during World War II a while ago and there was an interesting information in there: apparently many kapos (who were head prisoners) had young boys as lovers and they often dressed these boys in female clothing. Since these boys played the same passive role as women do in heterosexual relationships and wore women's clothing their bodies started to slowly transform and many of them grew much larger breasts. So the point is: the development of some human body features is directly connected not only to our biological sex but also with the roles we play in society and in a sexual relationship. And if Peter was raised as a girl for so many years by his crazy aunt his body would also gain some lady features.

Also, there are cases of faux pregnancies, where the person wants to be pregnant with such desire that the body produces the hormones as if it was with child. Breasts get larger, stomach starts to pudge, and even some lactation occurs. So its in the realm of plausible that he could have some hormone production that is produced to give him a more fuller chest. Not to mention, that he could have Gynecomastia due to the psychological distress he has had from his being raised as a girl.

Her aunt said she was a doctor, so maybe she was able to get her (him?) on hormones?

I thought about this too.
After the film was over and the secret was revealed I thought "wait, I think Angela had some boobs coming in..."
So I went back and looked at certain scenes and yeah she does.
But this is nitpicking I suppose.
You could also say that she/he doesn't have a bulge in tight shorts where there should be one. Lol

There's a scene where Angela and Ricky are sitting next to each other in a bench and they're both wearing those short shorts. Totally 80's style. And Ricky's balls are practically falling out Lol with a normal bulge for a boy. But Angela? Nothing- like you would expect from a GIRL.
So yeah there's things like that but if they made her flat chested with a big bulge in her pants the audience would be thinking WTF is that about?!
