And I think after his first heart attack, you'll notice that Jessica Lange's veil comes off slowing signifying that he's that much closer to death...
Also, I think when Joe says that something is wrong, there is a momentary shot that show's jessica through a distorted mirror, kind of like maybe he was stroking out or just having heart failure...
very interesting his dance with death throughout the whole movie....
