I just saw the two part episode "Mystery Man" where Banner has a face mask as he helps McGee escape the forest. It was hilarious. I've never seen something so contrived. The writers were very creative to think of such a plot.
But it was way too contrived though. Banner did not have burns on his face after losing his memory. And when McGee finally gets to meet him, his face is covered in a mask. Yeah right. How convenient. What a coincidence. lol
Then when the plane crashes in the woods, and McGee is hanging off a cliff, he passes out just as Banner changes into the Hulk. Another amazing contrived coincidence! I was laughing in disbelief at that.
Finally, at the end, he witnesses the masked man turn into the Hulk and realizes that the Hulk is a man who changes into it.
At that point, he should have easily deduced that Banner was the Hulk. When he last saw Banner in the pilot episode, Banner went into the lab and then it exploded and the Hulk came out. No trace of Banner was ever found either. So he could have easily added 2+2 to get 4. The answer was a giveaway at that point.
Plus the mystery man in the mask had Banner's voice, height, and hair. Anyone in McGee's position could have figured it out. It was already solved at that point. It was way too implausible for him to remain in ignorance.
McGee is a very single-minded and unimaginative man/reporter; he believes David is dead, so he is dead! Mind you, he was ready to believe David survived only when they(briefly) came face to face again in 'Broken Image'.
It is a surprise he did not re-recognize David as the "voice" of his "John Doe" from "Mystery Man" in "Broken Image", but as stated he is single minded.

...Or from talking to David on the phone in season 1's "The Hulk Breaks Las Vegas." Not going to give the story away for anyone who hasn't seen it or anyone that wants to review the episode for themselves, but suffice it to say Banner is forced to call McGee's hotel room from the hotel lobby. McGee repeatedly tells David he sounds very familiar. But naturally, McGee can't pinpoint that it is Banner.
What follows is suspenseful, as McGee leaves David on the phone to (try to) rush down the elevator to see this person he's been talking to on the phone.

There is a difference between what is obvious to someone who already knows the whole story, and what is actually obvious to somebody trying to piece it together from limited information.
Jack was inside the lab; David and Elaina arrived, and he hid. Jack hears them discussing the Hulk as though he were a third person ("if our friend comes back"). Jack had them under observation, and they weren't doing anything that could cause an explosion. They found Jack moments later, and David escorted him from the building. They left Elaina alone in the lab, in no danger whatsoever. Jack and David argued outside. The lab exploded, David ran in, the Hulk came out.
Jack quite reasonably believes that whatever caused the lab explosion happened AFTER he and Banner left. The "obvious" answer from Jack's POV is that the Hulk came back at that time and caused the explosion. Banner cannot possibly have caused the explosion, because he was outside with McGee: Therefore, he cannot possibly be the Hulk. There is no reason why such a possibility should cross Jack's mind. Unless, of course, he knows that he was the one who really caused the fire. But he doesn't, and his subconscious is likely to be real resistant to that idea.

Banner has "average" height and brown hair, like millions of people. Those don't even count as clues. When he is wearing the mask, you cannot see his hairstyle. You cannot make out the contours of his face. In fact, the mask makes it look as though his face is much fuller than it actually is. It isn't just that you cannot see his face; you actually get the illusion that when you take the mask off, you are going to see a guy who looks very different from David.
In the pilot, Banner carried himself like a respected professional dealing with a lowlife. His manner towards Jack was cold, aloof, and contemptuous from their first meeting. John Doe's manner towards Jack is respectful and grateful, almost submissive. When he starts to act with authority, it is the authority of a doctor treating a patient - caring and concerned. He and Jack like each other and show it. The whole psychological context of the encounter is completely different. As a result, David's tone, manner, and body language are also different.
So to summarize:
McGee has three brief conversations with Dr. Banner. Then Banner "dies" tragically, at which time McGee sustains a head injury and severe psychological trauma. Two years later, McGee meets a John Doe with average height and brown hair, who otherwise looks and acts totally different from Dr. Banner, but who has the same voice that Jack heard for ten minutes total two years earlier. This voice might sound vaguely familiar to Jack, but he and John already know that they have been to many of the same places over the past two years, which further misdirects Jack's efforts at placing the voice. Anyway, John turns out to be the Hulk, who Jack "knows" cannot be the Dr. Banner, because the Hulk was killing Elaina Marks while Banner was innocently talking to McGee outside the laboratory. reply share
If anything after his suspicions from the episode Broken Image, but more so episode Equinox. When Mcgee has David trapped in that fancy dress storeroom with the tranquilizer gun.
Even though David had that eye mask on, it should have been clearly the point, that Mcgee figured out David Banner was indeed still alive and the creature. Especially when a voice conversation, in an earlier season episode. Mcgee hinted the voice sounded very familiar to him. They probably could have had him calling out his name, with David instantly trying to deny it.
I have always wondered if Mcgee at some point eventually figured out Banner was the creature, however without any physical proof, the idea of Banner being the Hulk remained only a thought.

That's the point. No, McGee never knew it was Banner all along, despite all the "close calls".
As someone already pointed out, the CLOSEST call was when McGEE actually DID come face-to-face with Banner in "Broken Image". But the henchman of the villain that looked exactly like Banner was also there, and convinced McGee that it wasn't Banner.

Agreed. It really amazed me how AN INVESTIGATIVE REPORTER, who has been told on numerous occasions that he is good at his job couldn't put two and two together. He knew the Mystery Man had some medical training and that they were somehow connected in the past and he even recognized Banner's voice but couldn't put it together even after spending a week together.
Also in the episode "Homecoming", McGee told David's family that it couldn't be a coincidence that the Hulk showed up in Banner's lab at the time of the fire and also around his sister's lab around the time of her lab being damaged. At the end of the episode David's sister invited McGee to stay for Thanksgiving dinner and said that there is already a place set at the table because they had a relative that couldn't make it. Hmmmm
Only a handful, but probably enough people know that McGee should be noticing a pattern, everywhere I go where someone sees the Hulk, there's always some guy named 'David' around, it's not that big of a logical leap for him then to figure out just who that 'David' really is.
In the comics since Tales To Astonish everyone knows Banner is The Hulk, it's too much of a stretch for people not to know, there's no secrecy in every Hulk adaptation since the 96 cartoon.
Him not being able to figure it out and notice there always someone called David when ever there's a Hulk sighting makes him the biggest moron in the world. They always didn't want too many people to know Banner is the Hulk, fear was once they did the show was gonna take a turn for the worst.
Yeah, he should have recognized "John Doe's" voice as David Banner's voice...
Also as noted by a few other posters here... Everywhere the Hulk is sighted, there is a man named "David B." there too. How many "David B.s" can there be, before someone should realize there are too many of them to be a coincidence?
Then you have the episode "Homecoming"... The Hulk, who "killed" David Banner, just happens to be seen, in the town of David Banner's childhood home, where his father and sister are... Jack McGee even mentions he thinks there is a connection, but then doesn't wonder who the male relative was who didn't show up for Thanksgiving dinner.
You also have "John Doe" & "The Hulk" coming into existence just before David Banner "dies", and David Banner has no body to bury.
Looking at all those pieces of the mystery... Jack McGee should have put them together but didn't. I guess, Jack McGee is not very intuitive...
Too much of a stretch of credibility for people not to know Banner is the Hulk. They had to have a lot of secrecy about Banner is The Hulk, in the comics everyone knows Banner is The Hulk which at the time thought would murder the whole show if they did that.
