shareHarding is there because he is homosexual which in those days was considered a mental health disorder that could be cured. Virtually all the active men on the ward are there because they are not conventional in some way (epilepsy, anxiety, submissiveness etc) and the movie is saying that people who are unconventional in America are treated as pariahs, segregated and "worked on" by the system.
