3. There could easily have been other alarm activators besides the one involving the vault key. One of the tellers could have secretly activated it.
4. When Sal first showed his gun to the manager, the latter could have said something to alert the person he was talking to that a robbery was happening. A common technique is to answer the person's sentences with completely irrelevant statements. For example...

Person: Excuse me??
Manager: Our branch in Manhattan handles the calendar.
