How were the police actually alerted to the robbery? The man who saw the smoke was told everything was fine by the bank manager.

1. The man who saw the smoke detected odd body language by the bank manager.
2. The woman who spoke to her boyfriend on the phone may have used a code word or phrase to tip him off that something was wrong.
3. There could easily have been other alarm activators besides the one involving the vault key. One of the tellers could have secretly activated it.
4. When Sal first showed his gun to the manager, the latter could have said something to alert the person he was talking to that a robbery was happening. A common technique is to answer the person's sentences with completely irrelevant statements. For example...
Person: What would my interest rate be?
Manager: Next year.
Person: Excuse me??
Manager: Our branch in Manhattan handles the calendar.
Person: What?? Is something wrong?
Manager: Yes
Person: Should I call the police?
Manager: Yes, please, if you don't mind.
