I don't understand, please help me!

I watched this movie last night. I had fairly high hopes, because I first read about the movie in Roger Ebert's The Great Movies. I was not really frightened by what I saw, which can probably be chalked up to de-sensitization from modern horror. However, I really enjoyed the cinematography and the direction.
It was really well done, and I though I was following everything pretty well...then the ending happened.

I learned from Wikipedia that Sutherlan's character was having premonitions of his own death the whole time. That makes sense because of the drowning incident where he knew something was wrong. But where did the dwarf come from? Was she the third sister? Was she just a random psychopath? Were they plotting to kill him, or did it just happen?

please help me!

There is no nefarious plot to do away with him: there was a serial killer terrorizing Venice, and Sutherland's visions of his death and funeral set into motion the events that make them come true, which cause the visions and so on. It's like a loop in a time travel film.
