shareI'm not certain, but I think Melvin got the disease from the women in the first explicit scene in the film. If you check out Mario's "Baadassss" the biopic about Melvin during the making of Sweetback, it sort of eludes to that.
Personally, I don't believe the sex was real for the simple fact that Melvin wasn't making a "real porno," just trying to disguise it as such to keep the unions off his back. Plus, the sex scenes in Sweetback are mostly just Melvin laying motionless on top of various women throughout the film. Seemed very artificial to me. I could be wrong though.
