I think this has been touched on before, but the devil voice was Melissa's (and then Jody's) own voice, as if "the devil was already within" the whole time.
I think what will happen after the end of the movie is that the cycle will begin anew. Melissa saved her sister from dying (for supposedly all time until Melissa took her life -- we can see the moment of Melissa saying "Die, Witch!" as Melissa releasing her from her mortal coil), although Lucinda would continue to age indefinitely.
Now that Jody has pacted with the devil, Jody is now in control of Melissa's life, and Melissa will now age indefinitely, and she will probably go mad just as Lucinda did. Just because your body won't actually die doesn't mean that the whole thing just won't go stright down the toilet, especially the mind.
As far as who Luther and Molly are, I suspected that they are some sort of distant cousins, as "Strickland" is a pretty common name. Luther and Molly are obviously aware of Melissa, and in the unedited version, Molly is even supportive of Melissa having a relationship with Jody.
Are they trapped on the walnut ranch? Probably not. Melissa could have statyed there this whole time as she, as a woman, did not have the freedom to move about like modern women do. She also had that big expanse of land (once her actual parents passed on)with no one to work it until Luther and Molly showed up. Jody might have taken her back to Austin in his snazzy car (once the vomit was cleaned up).
Anyway, I'm now done with all my parentheticals. I really like this movie, although I will admit it had more interest for me when I thought that Don Henderson and Tom Laughlin were the same person. XD
