If by "hack" you mean when Irene is going to call one right before they sing "Elegance," I always assumed it was like a taxi. I was in a production of this show, stage version obviously, about 10 years ago, and that's how the line was delivered.
As far as how long Mr. Levi had been dead, in the stage version, it's at least 10 years. Dolly is a middle aged woman in the stage version, so this timeline would make sense.
