shareThe members of the AMPAS (Academy of Motion Picture Arts & Sciences) each receive a ballot. Just like any other election, the votes are tallied, and the award goes to the winner. A tie is unlikely, since there are usually 4 or 5 nominees in each category. However it is possible.
It's been many years since I've seen the video clip of the 1968 Best Actress Streisand/Hepburn tie, but I do recall the surprise in presenter Ingrid Bergman's voice/facial expression at the time. Also, as I recall, Hepburn didn't show up for the ceremony. Streisand was the winner who actually made an acceptance speech.
As a matter of fact, before she went on to present the award for Best Actress, Ingrid Bergman was warned to read the ENTIRE contents of the envelope when she opened it. She thought those were rather strange instructions to be given before going on to present the award but she was still caught by surprise when opening the envelope revealed the first tie since the 1930's.
