Elizabeth could have easily made her presence known

the last shot shows Elizabeth hapless and bound in the chamber forever (mouth gagged)

even if the mouth is gagged , nearly all human beings can produce (loud)sound though the Larynx-thorax. try it, its easy ;)
she could have gone for this , attracted attention and got herself saved rather than from rotting there for the rest of her life.
was this in the book too ? or was it the director's own 2 cents to create the shock and awe ?

Good point. I hadn't thought of that.

The thing I noticed was it didn't seem like there was any moment where Nicholas could have put the gag on her. I mean, if I recall correctly, he simply shoved her into the sarcophagus.

She was in a device full of metal spikes. So not only probably bleeding to death anyway but realizing the body would be like that forever.

The last shot shows her hands are free as her fingers are holding onto the small window in that iron maiden thing, so it’s really perplexing why she didn’t just remove the gag and yell.
