Mary is simply crying out for attention and playing the role of the victim. Children learn early no that adults tend to overlook any wrong doing the instant the kid is shick. Especially if said child is spoiled rotten.

She isn't ill. While it isn't completely unknown for a child to have a heart attack in extreme circumstances, Martha's aunt is a self-serving idiot. Mary is just playing it up for attention, manipulation or distraction- depending on her motive.

Let's put it this way, Joe's prescription of giving her a whack on the butt isn't to cure anything physical.
