Near the beginning, when Isak and Marianne get to the house on the lake, we see the flashback with his brother hitting on their cousin and we later find out that they are now married and have a baby.
Am I missing something?

Well, the only thing you may have missed is that cousin marriages used to be pretty common in Northern Europe at the time (and they're still legal in most of the world).

And Marianne called Isak Uncle as well. So perhaps she and Evald were cousins as well.
