He was a heavy drinker. I don't know if he also smoked but one or both can really age you. His drinking indirectly killed him.

He also spent a great deal of time in Africa without sun screen and of course, the sun ages a person. Whether Holden looked "old" in Kwai is a moot point. He was better looking than anyone else of that day and age, or anyone else in that film!
