My interpretation of the movie is this: the old man died and the final scene shows the happy family in the heaven (taking on account that the boy had a flashback when he met the girl passing over that bridge). Any opinions?

The final scene is simply a montage of several previous scenes in the movie, depicting and summarizing Huw's "precious memories" of the good times amidst the bad and his bond with his family.
