I believe Alex is still involved with Donate Life but I don't think he is as active as he used to be when he was doing Three Rivers. I think it was last year, he was a big supporter of Taylor's Gift which is another Organ Donation awareness charity. www.taylorsgift.org He still posts things about Donate Life on occasion on his facebook account but he isn't very active on facebook. He was also at University of Hawaii's Relay for Life last year, not exactly sure how involved he was but I believe he was a speaker at the event. I only saw pictures and tweets about this.
In February he hosted Sustainable Coastlines "I heart the Ocean" fundraiser, again not sure how involved he is with this charity other than hosting the event.
Last year he spent Thanksgiving visiting sick kids at a hospital in Hawaii. Again, I only saw tweets and pictures of this, no official article that I have seen.

He is also a benefactor of the Heath Ledger Scholarship which is an award funded by the Australians in Film which is a LA bases non profit organization, its the industries guild for Australian Filmmakers and performers in the US.

He's done some other charity stuff like Surf4Hugs in Hawaii and he was going to do a Tough Mudder charity event with the Stray Dogs team in Lake Tahoe last year but had to drop out due to scheduling conflicts.
I don't know if these are the answers your looking for but it was all I could come up with. I think Alex is probably involved with more charities that we know but I would guess they are more local charities in Hawaii and he tends to fly under the radar so it probably isn't reported.
