Um yeah, it's not really kidney failure. He gained weight specifically for the show eating 5000 calories a day. Google it.

He didn't "get fat", he put on mass to make his body match his avatar's. You can hear the whole story in the Season 7 episode "How Mac Got Fat".

In real life, Rob did it as a way of criticizing the way most actors appear to get better looking in later seasons of their shows because the budget goes up for makeup. So he decided to gain weight, grow an unsightly beard, and make himself look as unappealing as possible.

He encouraged the rest of the cast to join him, but they said no (no, and also "that's disgusting").
