Both young ladies died of a heart attack. In Elliot's case, the ham sandwich was a lie. Her heart failure was due to a combination of an unhealthy lifestyle and bad genetics.
While Karen Carpenter did suffer from anorexia, she was mostly recovered from that. Karen was not starving at the time she died - the heart damage her earlier anorexia and bulimia caused was permanent. Her damaged heart actually couldn't support her after she gained weight. She also died by a heart attack.
Damn. Fucked up.
